چکیده

Reconstruction of aortoiliac segment and femoroperoneal graft. Such an extensive operation is now sometimes indicated. The number ofmajor amputations done each year for vascular insufficiency is not precisely known but is probably about 10 000. This suggests that between 20% and 40% of all critically ischaemic legs are amputated eventually. Society's attitudes to amputation have changed little since mediaeval times, and patients' responses to loss of a limb remain nearly as dramatic as to the announcement of terminal illness. Recent advances in vascular surgery have led us to reappraise the optimal management of critical leg ischaemia and in this article we consider the roles of amputation and revascularisation.

Revascularization or amputation.

The factors which determine the choice, revascularization or amputation of an ischaemic leg are very numerous, variable and sometimes also related. They are concerned with the extent and course of the gangrene, the general condition of the patient and the risk of operation, the technical operability in terms of arterial reconstruction, the skill and judgement of the vascular surgeon, the motiva...

Perioperative and rehabilitative outcomes after amputation for ischaemic leg gangrene.

INTRODUCTION Amputation of the severely ischaemic leg is often done when limb salvage is not possible or the patient is not fit enough for it. It is therefore important to determine the natural history of these amputees as our understanding of this will have significant impact on decision making. The aim of this study was to determine the early and late morbidity and mortality rates and rehabil...

The impact of diabetes on current revascularisation practice and clinical outcome in patients with critical lower limb ischaemia.

OBJECTIVE To compare current revascularisation practice and outcome in diabetic and non-diabetic patients presenting with critical limb ischaemia (CLI) to a single vascular surgeon. METHODS Data for 113 patients presenting with CLI were collected prospectively over a 3-year period. Forty-four (39%) were diabetic. Treatment was classified as percutaneous angioplasty, arterial reconstruction, p...

Strategies for free flap transfer and revascularisation with long-term outcome in the treatment of large diabetic foot lesions.

OBJECTIVE/BACKGROUND To analyse the impact of ischaemia and revascularisation strategies on the long-term outcome of patients undergoing free flap transfer (FFT) for large diabetic foot lesions penetrating to the tendon, bone, or joint. METHODS Foot lesions of 63 patients with diabetes (median age 56 years; 70% male) were covered with a FTT in 1991-2003. Three groups were formed and followed ...

ABC of vascular diseases. Rehabilitation of the amputee.

A successful above knee amputation. The patient was walking on crutches within 10 days of operation. He had run eight miles a day until acute thrombosis of a popliteal aneurysm produced irreversible ischaemia. Most lower limb amputations are now done because of vascular insufficiency. In 1987, 4985 new lower limb amputees were referred to the disablement service centres (previously known as art...
